Risk Factors (Braden Scale)

Each patient can be assessed for risk factors using the Braden Scale to track their clinical risk factors over time. The score is applied to a patient, and is consistent no matter what wound is being managed.

•  Open the Risk Factors page by tapping the “Risk Factors” button on the Patient Summary.

• On the Risk Factors page, users can view a patient’s current and past Braden scores, and that patient’s clinical assessments.

• To create a new Braden scale score, tap the “+” symbol in the top left of the screen.

• Select a value for each field of the assessment, and press “Save” to add the Braden Scale score to the patient’s data.

• If more information is needed on a scoring criteria, simply tap “Read more under the score option.

• Please note that Braden Scale scores cannot be saved unless all items are filled.

• Clinical assessments can be saved by selecting the desired option, and pressing “Save” to store the data.
