Documenting Assessments

• The assessments page shows a listing of all assessments for a wound.

• Create a new assessment by pressing the “+” button in the left corner of the page.  To prevent duplicate entries, assessment records are limited to one entry per day.

• To enter assessment parameters, click the “+” icon next to each field to expand each field. Fields marked with an asterisk denote clinically recommended parameters.

• This expands the field to show all available options.

• PUSH score (only applicable to Pressure Ulcer type wounds), is calculated automatically.

• 6. The Notes feature provides the ability to automatically generate a text summary of the assessment. The Copy feature can be used to copy this assessment summary to any text editor.

• Press “Save” after filling all relevant fields to save the assessment.

• The user can pan the table left or right to show more information about an assessment.
